Exemestane (commonly known by its brand name Aromasin) is an oral steroidal, irreversible Type I Aromatase Inhibitor (AI). It is primarily prescribed for the treatment of hormone receptor-positive early and advanced breast cancer in postmenopausal women.
In hormone optimization, testosterone replacement therapy (TRT), and performance settings, Exemestane is widely utilized to control elevated circulating estrogen (estradiol) levels. Unlike non-steroidal aromatase inhibitors (such as Anastrozole/Arimidex or Letrozole) which temporarily bind to the enzyme, Exemestane acts as a suicide inhibitor. It permanently deactivates the aromatase enzyme, preventing it from converting circulating androgens (testosterone) into estrogens until the body synthesizes new aromatase enzymes.
Key Benefits & Applications
-
Potent Estradiol Reduction: Efficiently lowers circulating serum estradiol ($E_2$) levels by up to 85%, mitigating symptoms of hyperestrogenism.
-
No Estrogen Rebound: Because it permanently destroys the bound aromatase enzyme rather than reversibly blocking it, there is no sudden surge or “rebound” of estrogen when dosing is stopped.
-
Mitigation of Estrogenic Side Effects: Prevents water retention, gynecomastia (breast tissue enlargement), hypertension, and bloating caused by excess aromatization of testosterone.
-
Support for Endogenous Testosterone: By lowering central estrogenic feedback at the pituitary level, Exemestane can stimulate an increase in endogenous Luteinizing Hormone (LH) and Follicle-Stimulating Hormone (FSH) release.
-
More Favorable Lipid Profile: Compared to non-steroidal AIs, steroidal AIs like Exemestane generally show a slightly milder adverse impact on HDL (“good”) cholesterol levels, though lipid monitoring is still necessary.
